How’s your relationship with alcohol?

Two paths: a short check-in for you (for people reflecting on their own use), and a separate family-based screening if you are concerned about someone else. Not a diagnosis.

How it works

  1. Choose who this is for using the buttons at the top. Questions differ for your use versus what you’ve noticed in someone you care about.
  2. Get one of three bands (lower / mixed / higher concern)—educational only. A clinician decides what care fits.
  3. One clear CTA: book a free 15-minute assessment with our team when you’re ready—no pressure.
Crisis or withdrawal? If you are in immediate danger, call 911 (or your local emergency number). In the U.S., call or text 988 for mental health crisis support. Alcohol or benzodiazepine withdrawal can be medically serious—seek urgent in-person care if you or someone you care about may be dependent.

The check (for you)

Six questions. One at a time.

1 of 6
In the past month, how often have you used alcohol or substances in ways you later regretted?
How often have you missed or shortened work, school, family, or personal commitments because of use or feeling off the next day?
How often have you felt you should cut down—but didn’t or couldn’t?
How often have others expressed concern about your drinking or substance use?
How often have you used alcohol or substances to manage stress, anxiety, loneliness, or boredom?
Overall, how satisfied are you with how things are going in this area of life?

Family-based screening (what you’ve noticed)

Ten yes/no questions. One at a time. This does not diagnose another person.

1 of 10
Have you noticed your loved one using substances more often or in larger amounts than they intend?
Have they tried to cut down or stop using but seem unable to?
Do they spend a lot of time obtaining, using, or recovering from substances?
Do they continue using despite problems at work, school, or home?
Have you observed continued use despite relationship conflicts or arguments caused by substances?
Do they give up or reduce important activities because of substance use?
Do they seem physically or emotionally different (e.g., mood swings, sleep changes, irritability) when using or not using?
Have you felt the need to confront them or suggest they cut down?
Do they become defensive, irritated, or dismissive when substance use is brought up?
Has their substance use negatively affected you or your household (stress, finances, safety, emotional impact)?

Common questions

Who sees my answers?

In this draft, answers stay in your browser until you submit—and nothing is sent to Sobio unless you later connect a real email capture (not wired here). Production should use HIPAA-compliant tooling and a clear privacy notice.

Can I do this for a family member?

Yes. Choose I’m concerned about someone else and use the family-based screening (what you’ve noticed). The short check for yourself is meant for the person reflecting on their own use. Neither replaces a professional assessment of another person.

Why don’t you ask for email first?

You should understand what you’re getting before sharing contact info. If you want email reminders or a PDF later, that can be optional after results—Sobio’s approach favors trust over pressure.
